Petitioner is Judith Ann Baker, an adult individual residing at 4702 Courtland Street, Camp Hill, Cumberland County, Pennsylvania, 17011. 2. Petitioner has resided at the residence specified in Paragraph 1, above, for the five year period prior to the date of the Petition. 3. Petitioner intends to change and requests this Court issue an Order changing her name from Judith Ann Baker to Isabella Grace Kurtz. 4. Petitioner desires to have her name changed to Isabella Grace Kurtz for the following reasons: a. By changing to the surname of Kurtz, Petitioner would be using her maternal grandfather's last name, thereby using and maintaining the family surname of a man who she knew, loved and respected. 5. There are no judgments or decrees pending against Petitioner and there is no child I whose name may be affected by the proceedings. WHEREFORE, Petitioner requests this Court to: A. Set a date for a hearing on the Petition in accordance with 54 Pa.C.S.A. Section 70 1 (a. 1)(3)(i); and B.
