AND NOW, this I~ day of -2~r1d~l , 2012, .upon consideration of
the Petition for Appointment of a Permanent Plenary Guardian of the Person and Estate
of Paul W. Kunkle pursuant to 20 P.S. Section 5511, and following a hearing at which
evidence was presented by the Petitioner in support of the Petition, and, based upon the
foregoing, it is hereby ORDERED and DECREED:
1. That PAUL W. KUNKLE is an incapacitated person in that he suffers
from a mental impairment that prevents him from receiving and evaluating information
effectively and prevents him from formulating and communicating decisions to such a
significant extent that he is unable to-manage his financial affairs or meet essential
requirements for his physical health and safety on a daily basis, and is adjudicated as
such.
2. That the Court appoints Kim E. Rutz as Permanent Plenary Guardian of
the Person and Estate of PAUL W. KUNKLE.
3. That the Guardian shall file on a timely basis with the Court all reports and
accounts required by statute.

4. That the Guardian shall be entitled to charge a guardianship fee of $150.00 per
month for the usual and ordinary services required by that position and an additional
$50.00 per hour in the event she is required to provide any services outside the scope of
routine guardianship care.
