CHANGE OF NAME OF MINOR w Tom. PETITON FOR CHANGE OF NAME OF MINOR AND NOW, comes Petitioner, Tabitha Morrow, by and through her attorney, Drew F. Deyo, Esquire, and in support of her Petition for Change of Name of Minor, makes the following declaration: 1. Birth Date and Place: The minor child, Bryanna Lyn Holley, born January 31, 1999, was born in Cumberland County, Pennsylvania. A certified copy of the minor's birth certificate is attached to this Petition. 2. Birth Name: The minor was named Bryanna Lyn Holley at birth, and as of the date of this Petition, the minor has always been known by that name, with the following exceptions, if any: N/A 3. Residence: The minor child resides primarily with her Grandparents at 833 Pheasant Drive, Carlisle, Pennsylvania 17013. 4. Minor's Parents: The mother of the minor child is Tabitha Morrow, Petitioner, and she resides at 136 South Lockwillow Avenue, Harrisburg, Pennsylvania 17112. Mother shares physical custody of the minor child with Mother's parents, William Morrow and Susan Morrow, who both reside at 833 Pheasant Drive, Carlisle, Pennsylvania 17013. Both William and Susan Morrow concur with Mother's Petition to have the child's last name changed to Morrow. Reason for Name Change: Petitioner seeks to change said minor's last name to Morrow as opposed to Holley, and offers the following reason for seeking to do so: All those who care for the minor child share the last name Morrow, and the minor child is the only member of her family with the last name Holley, which is Petitioner's maiden name. Petitioner, therefore, wants her child to share the same last name as the rest of the child's family. 6. Legal and Creditor Status: The minor child does not have any outstanding judgments, and has never been convicted of a crime. 7. Additional Declarations: Mother plans to run notice of the minor child's potential name change in two local newspapers pending a hearing in this matter. WHEREFORE, Petitioner respectfully requests that this Honorable Court order the minor's name be changed as follows: Bryanna Lyn Holley to Bryanna Lyn Morrow.
