North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owner of the properties against which this lien is filed is TSL Property
Management, LLC, with its principal office at 2 Pemberton Drive, East Berlin, PA 17316-9319.
A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 1400 Newville Road, Carlisle, Pennsylvania 17015.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "B" having a tax parcel number of 29-19-1653-023 and known as Evandale Court Mobile
Home Park on Newville Road, Carlisle, Pennsylvania 17015.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid water and sewer user fees pursuant to an agreement between the
parties against the above-described properties in the amount of $32,052.91, plus any additional fees
incurred thereafter.

Interest is claimed on the above-mentioned fees until paid in full.

thence by same, North 77 degrees 15 minutes West 73.4 feet to a point, the Place of
BEGINNING.
HAVING erected thereon a house known as and numbered 1400 Newville Road,
Carlisle, Pennsylvania 17015, together with a former garage now a separate apartment
building known as 1400 Newville Road, Apartment No. 1, Carlisle, Pennsylvania 17015,
and also including a supply and laundry building, and improvements for mobile home lots.
BEING the same premises which Jack Britton Cooper, also known as Jack B.
Cooper Sr., and Faye B. Cooper, husband and wife, by Deed dated January 15, 2002 and
recorded February 25, 2002 in the office of the Recorder of Deeds, in and for Cumberland
County, at Carlisle, Pennsylvania, in Deed Book 250, Page 2505, granted and conveyed to
Sandy Point Properties, Ltd., a Pennsylvania Corporation, Grantor herein. The said
recording, was a rerecording of the Deed that had been previously recorded to Cumberland
County Deed Book 250, Page 142.
AND the said Grantors hereby covenant and agree that they will warrant
