TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for x,5897.70 together with costs
upon the following claim:
Plaintiff acquired the obligation originally issued by First National Baok,:of Omaha. Plaintiff demands of Defendant the
unpaid prinicipal balance due under the Acct. number ending in 3727. There is due and owing from defendant the
sum of $5012.71, plus six percent (6%) statutory interest calculated in accordance with 41 P.S. Sec. 202 in the
amount of $884.94, for a total of $5897.70. See attached Verification (Exhibit "A").
In accordance with the Fair Debt Collection Practices Act, 15 U. S.C.A. 1692(g)(a) on August 22, 2011, Plaintiffs
counsel sent a written notice to Defendant. To date, no response has been received. (See Exhibit "B").
