Haggerty, Esquire, of ABOM & KUTULAKIS, L.L.P., respectfully request that This Honorable Court allow the minor child to change his name to Casey Evan Sites, and in support thereof avers as follows: 1. Petitioner is Angel Sites, on behalf of her minor child, Casey Evan Stitely. 2. The child's date of birth is March 19, 1994 and he is currently seventeen (17) years old. 3. Petitioner has primary physical custody of the child. 4. The child resides with Petitioner at 61 Walnut Dale Road, Shippensburg, Cumberland County, Pennsylvania 17257 5. At the time of his birth, the child was given the name Casey Evan Stitely. 6. Petitioner desires to change the child's name to Casey Evan Sites. 7. Petitioner desires to change the child's name for the following reasons: a. Petitioner has had primary physical custody of the child; b. The child does not have his father's last name but rather has his mother's maiden name. c. Mother married ten (10) years ago and her married name is Sites. 0-7«tFj d. Stepfather has been acting as the child's father since he was little„ and the biological father has not been involved. e. The Child has three (3) siblings that all have Sites as their last name. f. The Child does not wish to be associated with his father as they have had a very estranged relationship. 8. Over the past five (5) years, the child has resided in the following locations: a. January 2005 to July 23, 2008 - 184 SME, Shippensburg, Cumberland County, Pennsylvania 17257 b. July 23, 2008 to present - 61 Walnut Dale Road, Shippensburg, Cumberland County, Pennsylvania 17257. 9. There are no judgments or decrees of record against the child. If This Honorable Court requires proof of such, Petitioner will provide this at the hearing on this Petition. 10. Petitioner will provide notice of the filing of this Petition and the hearing; on this Petition in the Cumberland Law Journal and The Sentinel. Petitioner will provide proof of such at the hearing on this Petition. 11. Attached to this Petition is an original fingerprint card containing the fingerprints of the child. The child's fingerprints were placed on the fingerprint card on February 20, 2012 at the Shippensburg Borough Police Department, Shippensburg, PA 17257. 12. The non-petitioning parent of the child is Rodney Brookens. Petitioner will provide notice of the filing of this Petition and the hearing on this Petition to the non-petitioning parent.
