The Petitioner Matthew Allen Thumma, by and through his attorneys, Andrews and Johnson and
Ronald E. Johnson, Esquire, request that this Court Order a change of name for him pursuant to 54 Pa.
CONS. STAT. Section 701 et. seq. and in support thereof respectfully represents as follows:
1. Petitioner is an adult individual having been born on July 9, 1986.
?. Petitioner Matthew Allen Thumma, is a resident of Cumberland County, Pennsylvania and
has resided at the following locations during the five years next preceding the filing of this petition:
463 Park Drive, Carlisle, PA 17015 (19 years); and
3. Petitioner requests that his name be changed from Matthew Allen Thumma to Matthew
Allen Helm.
4. The reason for the requested change of name is that petitioner has resided all of his life with
his mother, Paula Helm. He has not spoken to nor has he had any contact with his father, Douglas
Thumma for a period of approximately 7 years. He has very little contact with his paternal grandparents
and that which he has had has resulted in conflict between them. He is much closer with his mother and
has resided with her all of his life and desires to change his last name to that of his mother.

WHEREFORE, Petitioner respectfully requests that this Court enter an Order changing his name
from Matthew Allen Thumma to Matthew Allen Helm.

AND NOW, this 26 h day of March, 2012, upon consideration of the attached
Petition for Change of Name, a hearing is scheduled for Thursday, May 24, 2012, at 1:30
p.m., in Courtroom No. 5, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioners shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 [Historical and
Statutory Notes] [Main Volume]), and 54 Pa. C.S. §§701 et seq.

AND NOW, thisay of?? 2012 after hearing upon the within Petition and upon
motion of Ronald E. Johnson, Esquire, Attorney for Petitioner, and upon presentation. of proof of publication
of notice as required by law, a report from the Pennsylvania State Police, together with proof that there are
no judgments or decrees of record or any other matter of like effect against the Petitioner, and it appearing
that there is no legal objection to the granting of the prayer of the Petition,
IT IS ORDERED AND DECREED, that the name of the Petitioner be and is hereby changed from
Matthew Allen Thumma to Matthew Allen Helm.
