The facts upon which this claim is based:
This claim is based on medical services which were provided by Holy Spirit
Hospital for the dates listed above. Bills were issued directly to the patient decedent for
these services since he was uninsured. We note that the August 2011 hospital
admission occurred within six (6) months of the decedent's death and, therefore, this
admission and bill totaling $29,847.70 qualifies for priority treatment under the Probate
Estates and Fiduciaries Code 20 Pa.C.S.A. § 3392, which gives payment priority for the
costs of medicines, medical, nursing and hospital care furnished to the decedent within
six (6) months of his death.

6) Date of Death: November 5, 2011
7) The claim arose prior to the death of the Decedent.
