claims a construction lien against the below-stated property for work, services, materials and/or equipment performed or
furnished in accordance with a contract with: G & G EQUIPMENT SPEC. LLC
2. The real property subject to this lien claim is identified as: PARCEL ID: 10-22-0529.005 and also known as: FIRST
INDUSTRIAL 301 RAILROAD AVE (PREMIER PROJ.# 201158) SHIREMANSTOWN, PA
3. This Lien is claimed against the interest of: FIRST INDUSTRIAL PENNA in the real property.
4. The work, services, materials and/or equipment described as SUPPLIED BATHROOM PARTITION MATERIALS were
provided pursuant to the terms of a written contract (or in the case of a supplier, a delivery or order slip signed by the owner,
contractor or subcontractor), dated 12/5/2011 between the claimant and G & G EQUIPMENT SPEC. LLC having an
address of 29 RADAM STREET P 0 BOX 59 SUMMEROALE, PA 17093.
5. The contract price of the contract under which this lien claim is made is $2,488.00.
6. The total of all payments received by the claimant against the contract price is .00
7. The amount of this construction lien is $2,488.00
B. The date of the provision of the last work, services, material or equipment for which payment is claimed is

9. If the claimant is a labor benefit fund, this lien is claimed only for labor benefit fund contributions, as such term is

person performed labor at such real property for which labor benefit fund contributions are unpaid, and for each such
person, the amount of labor benefit fund contributions are unpaid for labor performed at such real property.
