Petitioners Mark Dion Engle and Lauren Carothers Engle, request that this Court order a change
of their daughter, Madison Grace Engle 's name, pursuant to 54 Pa. Cons. Stat. Section 701, et seq., and
in support thereof makes the following the averments:
1. The petitioners were born on 06/27/1983 and 05/24/1983, respectively, and are currently 28
years old. The petitioners' daughter was born on 04/09/2005 and is currently 6 years old.
2. During the five years preceding the filing of this petition, the petitioners' daughter has resided in
the following places:

3. There are no judgments or decrees pending against the petitioners or the petitioners' daughter.
4. The Petitioners request that their daughter's name be changed from Madison Grace Engle to
Madison Grace Nickey Engle.
5. The reasons for the requested change of name are the petitioners would like to acknowledge
the support of Madison's grandparents and honor them through the inclusion of their last name Nickey.
WHEREFORE, the petitioners respectfully request that this Court enter an Order changing their
daughter's name from Madison Grace Engle to Madison Grace Nicker Engle.

consideration of the Petition for Change of Name, and after havi:
had a hearing in this matter where the parents of the child
presented proof of publication as required by law in both a pape
of general circulation and in the Cumberland Law Journal, and it
being their desire to change the name of their daughter, Madison
Grace Engle, to Madison Grace Nickey Engle, the petition is here
granted. The child shall hereinafter be referred to in all
documents and in all manner as Madison Grace Nickey Engle.
