AFFIDAVIT OF ACCOUNT
The undersigned, being first duly sworn deposes and states the follows:
1. Yow Affiant is authorized by the Claimant as its Authorized Representative to make this Affidavit.
2. Yow Affiant has reviewed the account records of the Claimant with respect to the decedent. Yow
Affiant is familiar with these records and accounts and reviews them as a regular part of his/her
duties.
3. The Decedent pwchased merchandise in the amount of $ 53,163.87 evidenced by account
number 4705000240469296
4. The debt was originally held by Wells Fargo / Sacco !nc and assigned to
the corporation in the normal course of business by the original creditor or their lawful assignee. The
affiant states that to the best of his knowledge, information and belief there are no uneredited payments,
counter-claims or offsets against said debt. Said account has been assigned, transferred and sold to
CAVALRY PORTFOLIO SERVICES LLC with full power and authority to do and
perform all acts necessary for the collection, settlement, adjustment, compromise or satisfaction of said
claim. Further, the undersigned acknowledges that in making the assignment,
CAVALRY PORTFOLIO SERVICES LLC is now the owner of this account, and has
complete authority to enforce the rights of the original creditor with the debtor, and that the assignor or
original creditor has not further interest in said debt for any purpose
