AFFIDAVIT OF SERVICE
I, Jill M. Wineka, Esquire, being duly sworn, depose and say that by email dated and sent
January 31, 2012, true and correct copies of the Petition to Adjudge Ann B. Stum an Incapacitated
Person and to Appoint a Plenary Guardian for her Estate and Person; the proposed Final Order; the
Court's Preliminary Order scheduling the Hearing for March 16, 2012 and the Notice and Citation
were communicated to Ann B. Stum's son, Derek Stum to the email address of
derekncariCa)_yahoo.com. The email address was given to me by Derek Stum in a January 31, 2012
phone conversation I had with him. I received no notification that the January 31, 2012 email did not
reach Mr. Stum at the email address he provided to me. Attached is a copy of the January 31, 2012
email, (without the attachments), sent to Derek Stum.

Mr. Derek Stum:
Per our phone conversation of 1/31/12, I understand that you do not have a mailing or residential address to
provide me and that you do not have a land line phone. The cell number I used (954) 288-3582 is your girlfriend's
Carl's cell number. You asked me to email you the Petition; the. proposed Final Order; the Court's Preliminary
Order scheduling the Hearing for 3/16/12 at 1:30 in Courtroom 1 at the Cumberland County Courthouse; and the
Notice and Citation issued to your Mother about the purpose of the Hearing. All of the aforementioned documents
are attached to this email in three attachments.
As I explained, your sister Lisa has concerns that your Mother is not mentally able to care for herself or to
manage her financial affairs. She believes your Mother is incapacitated and needs to have a Guardian appointed
to be in charge of your Mother's living arrangements and healthcare and to be in charge of your Mother's assets,
income and debts.
The Court has appointed Attomey Mark Bayley to represent your Mother's interests at the Hearing. Attorney
Bayley can be reached at (717) 241-2446 and his office is located at 17 West South Street, Carlisle, PA.
Please let me know if you have any questions about the attachments or the 3/16/12 Hearing. Also, as I asked
when we spoke earlier, please acknowledge your receipt of my email and attachments. Thank you.
