PETITION FOR THE APPOINTMENT OF A GUARDIAN OF THE ESTATE
OF AMANDA HAMMAKER
AND NOW, comes the Petitioner and proposed guardians, David Hammake~'and
Suzanne Hammaker, by and through their counsel Reager & Adler, P.C. and respectfully
represents the following:
1. The name and address of the proposed guardians of the estate of Amanda
Hammaker, (hereinafter "the child") are David Hammaker and Suzanne Hammaker, natural
parents and adult individuals residing at 10 Dicken Drive, Marysville, PA 17053.
2. Amanda Hammaker is a minor child with a birthdate of July 18, 1989. The child's
birth certificate is attached hereto as Exhibit "A" and is incorporated herein by reference.
3. The child's natural parents are Suzanne Hammaker and David Hammaker both
residing at 10 Dicken Drive, Marysville, PA 17053.
4. The child resides with her natural parents at 10 Dicken Drive, Marysville, PA 17053.
3. The child is a beneficiary under an IDS High Yield Tax Exempt Mutual Fund
purchased by her late grandfather. Said Fund is administered by American Express Financial

Services and bears the account number 0000 011 5102 7691 5 002
4. The child's beneficial interest under said Annuity is approximately $71,696.00
6. American Express Financial Services requires that a court order evidencing the
guardianship of the estate be produced prior to funds being disbursed to a minor beneficiary.
7. The proposed guardians requests that they be court appointed as the Guardians of the
Estate of their daughter, Amanda Hammaker, so that her beneficial interests as referenced above,
may be released and held for her benefit.
8. The proposed guardians have knowledge that the child does possess a financial
estate of approximately $131,583.70, exclusive of the distribution being petitioned for hereunder.
An inventory of said assets is attached hereto as Exhibit "B"
10. The proposed guardians have no interest adverse to the child.
11. The proposed guardians and the child share the same religious affiliation.
