By way of a follow up to our several telephone conversations last month concerning the
above matter, I have enclosed the following:
1) The original, Court sealed Certificate of Appointment of Estate Trustee Without a
Will issued to Applicant, Ann Carolyn Dugas, surviving spouse of the decedent;
2) Affidavit of Cara J. Luther (as specifically requested); and
3) Affidavit of Ann Carolyn Dugas.
Please process these documents, and kindly issue a subpoena to the following entity:

A subpoena is required to obtain a complete copy of the police investigation and accident
reconstruction report of the accident of August 28 2009, in which Mr. Dugas was fatally injured.

I, ANN CAROLYN DUGAS, declare and state:
1. I am 59 years old. I was born on November 24, 1950.
2. I reside at 20066 County Road 18, R.R. #2, Williamstown, ON KOC 2J0.
3. I make this affidavit of matters within my own personal knowledge; if called as a
witness I could and would competently testify thereto.
4. After diligent search and inquiry, the Estate of Gilles Dugas, of which I am the
fiduciary is not, to my knowledge, indebted to any person in the Commonwealth of Pennsylvania,
nor have I been able to discover any such indebtedness.
5. Further, I will not exercise any power which I would not be permitted to exercise in
the jurisdiction of my appointment.
I declare, certify and affirm under penalty of perjury under the laws of the United States of
America that the foregoing is true and correct.

wrongful death of her spouse, Gilles Joseph Dugas, as the result of a motor vehicle accident on
