The Petitioner, Suwimon Sitthigarana, respectfully requests her name be chiuigid t6
Susan Sitthigarana, and represents as follows:
1. Petitioner, Suwimon Sitthigarana, date of birth July 19, 1978, wishes to have her first
name changed because she has gone by the name "Susan" for over twenty years, and was
nationalized as a United States citizen in 1998. Her current legal first name is not
common, hard to pronounce, and difficult to spell.
2. The Petitioner currently resides at 21 Carter Place, Carlisle, Pennsylvania, 17013. She
moved to this address at the end of February 2012. Prior to her current address,
Petitioner resided at 2 Rush Drive, Apt. 203, Carlisle, Pennsylvania, 17013 for two years.
For three years prior, Petitioner resided at 2320 Woodhollow Circle, Corona, California,
92881.
3. There are no judgments or decrees of like character of record against Petitioner.
4. Petitioner's Finger Print Card is attached to this Petition.
WHEREFORE, Petitioner respectfully request the Court, pursuant to the Act of
December 16, 1982, P.L. 1309, No. 295, § 2 (as amended), 54 Pa.C.S.A. § 702, for the
following:
(a) To schedule a hearing on this Petition;

(b) To direct that notice be given on this Petition and hearing; and
(c) To enter a decree changing the Petitioner's name from Suwimon Sitthigarana
to Susan Sitthigarana.

Petitioner will obtain certifications of actions filed of record from Clerk of Courts and
instruments of indebtedness from Prothonotary and Recorded of deeds dated no more than two
days prior to the hearing.

Petitioner's request for a change of name, and on presentation of proof of publication of notice as
required by law together with proof that there are no judgments or decrees of record or any other
matter of like effect against the petitioner, and it appearing that there is no legal objection to the
granting of the requested relief,
IT IS HEREBY ORDERED AND DECREED that the name of the petitioner be and is
hereby changed to Susan Sitthigarana.
