HomeMy WebLinkAbout04-5069Fl\misAknousepriarpe[namechange IN THE COURT OF COMMON PLEAS CUMBERLAND COUNTY, PENNSYLVANIA IN RE: SETH BRYAN PRIAR NOpY _ S06,7 .: CIVIL ACTION-IN LAW PETITION FOR CHANGE OF NAME AND NOW comes Petitioner Seth Bryan Priar, by and through his counsel STONE LAFAVER & SHEKLETSKI, and respectfully represents the following: Petitioner, Seth Bryan Priar, is eighteen years of age having been born on January 22, 1986, out of wedlock to his natural mother and custodial parent of Elizabeth Pauline then-Priar, now Knouse, and his natural father, Mark Anthony Rosati. 2. Petitioner, Seth Bryan Priar resides with his natural mother at 5033 Inverness Drive, Mechanicsburg, Cumberland County, Pennsylvania 17050. The petitioner has been a resident of Cumberland County since birth. 4. At birth, the minor child was named Seth Bryan Priar; Priar was the surname of minor child's natural mother, Elizabeth Pauline Knouse, formerly Elizabeth Pauline Priar. 5. Petitioner's birth certificate reflects his natural father's name of Mark Anthony Rosati, the same name to which Petitioner would like to change his name. 6. Petitioner is now a freshman in his first year of college attending the Maryland Institute College of Art. It is a four year program, for which the Petitioner hopes to receive a degree and later a certificate to teach. T The petitioner has never been convicted of any crime. 8. The petitioner has never filed for bankruptcy under I 1 U.S.C. § 101 et seq. 9. There are no judgements or liens of record and no actions pending against Seth Bryan Priar in this State or of the United States or of any government subdivisions. 10. The Petitioner, Seth Bryan Priar has not at any time made assignment through the benefit of creditors. 11. There are no claims, demands, liabilities, or obligations of any kind on any written instrument or otherwise against the Petitioner, Seth Bryan Priar under the name sought to be changed or any other name. 12. The Petitioner has no creditors who may be adversely effected or prejudiced by the proposed change of name. 13. Petitioner, Seth Bryan Priar desires to change his name from Seth Bryan Priar to Seth Bryan Roasti for the following reasons: a. Petitioner's mother, Elizabeth P. Knouse, placed the name Priar on the Petitioner's birth certificate because she was unmarried at the time of the birth and chose her maiden name as the last name. Petitioner's mother has since married and divorced and no longer shares the same name. b. Petitioner has enjoyed and continues to maintain a good relationship with his natural father and would like to have the same name as him even though his parents never married each other. C. Petitioner would like to have his name changed prior to receiving his diploma from college so that he may begin his professional life with his new name, beginning in the Spring 2008. WHEREFORE, Petitioner respectfully requests this Court to grant this Petition and direct the Petitioner's name be changed from Seth Bryan Priar to Seth Bryan Rosati.
