PETITION FOR APPOINTMENT OF GUARDIAN
FOR THE ESTATE OF MARY JANE OLIVER AND LEAUNA MARIE
OLIVER
NOW COMES the Petitioner, Emily M. Pivovarnik, the mother of the
minors, Mary Jane Oliver and Leauna Marie Oliver, who requests this
Court appoint a guardian of the estate of the minor persons, Mary Jane
Oliver and Leauna Marie Oliver, and in support thereof avers as follows:
1. Petitioner is Emily M. Pivovarnik, who is an adult individual
residing at 603 E. Keller Street, Mechanicsburg, Pennsylvania 17055.
2. Mary Jane Oliver is a minor residing with her mother, the
Petitioner, Emily M. Pivovarnik. Mary Jane Oliver was born on November
3, 2004.
3. Leauna Marie Oliver is a minor residing with her mother, the
Petitioner, Emily M. Pivovarnik. Leauna Marre Oliver was born on
February 18, 2009.

4. The relationship between the Petitioner and the minors is that
of mother and child.
5. Leo Oliver is an adult individual and is the father of the
minor children, the subject of this petition. Lee Oliver currently is
incarcerated at Green Correctional State Prison in Waynesboro,
Pennsylvania. He has no contact with the Petitioner nor the minor
children.
6. An appointment of the guardian of the estate of the minors
is necessary because the minors are named as beneficiaries of various
accounts by reason of a death of Carol Diane McAfoos, the godmother of
the Petitioner. The accounts and their values, as of the date of this
petition, are more fully set forth on Exhibit "A", attached hereto.
7. The name and address of the purposed guardian is as follows:
Richard L. Pivovarnik, 1010 Franklin Street, Carlisle, Pennsylvania
17013. Richard L. Pivovarnik is the maternal grandfather of the minor
children.
8. The written consent of Richard L. Pivovarnik to the
appointment is attached hereto as Exhibit "B".
9. No notice is required to be given to the United States
Veterans Administration or its successor.
10. Each of the minors, which are the subject of this petition,
are under the age of fourteen (14) years of age and are therefore not
the petitioners herein.

WHEREFORE, Petitioner requests this honorable court to appoint
Richard L. Pivovarnik as guardian of the estate of the minor persons
Mary Jane Oliver and Leauna Marie Oliver.

PETITION FOR APPOINTMENT OF GUARDIAN
Richard L. Pivovarnik, being duly sworn according to law, deposes
and says that:
1. Deponent resides at 1010 Franklin Street, Carlisle, Cumberland
County, Pennsylvania.
2. Deponent is the maternal grandfather of the minors, Mary Jane
Oliver and Leauna Marie Oliver.
3. Deponent consents to his appointment as guardian of the person
of the minors, Mary Jane Oliver and Leauna Marie Oliver.
