3) Creditor listed below is the owner and holder of a claim in the amount of
$848.00 as of January 3, 2012.
4) The facts upon which this claim is based: Notice of Reclamation filed for 2
payments of an annuity paid to Decedent, which funds were reclaimed by the
US Government after the funds were removed from the Decedents' Account
leaving a deficit in the account of $848.00.

Millersburg, Pa. 17061

AFFIDAVIT OF ACCOUNT
The undersigned, being first duly sworn deposes and states the follows:
1. Your Affiant is authorized by the Claimant as its Authorized Representative-
In-Fact to make this Affidavit.
2. Your Affiant has reviewed the account records of the Claimant with respect to
the decedent. Your Affiant is familiar with these records and accounts and
reviews them as a regular part of his duties.
3. The Decedent's Account was served with a Notice of Reclamation in the
amount of $848.00 for 2 annuity payments made to her account each in the
amount of $424.00 after her death. The $848.00 was withdrawn from her
account after Keystone Guardianship removed all funds from her account with
Halifax Bank.
