Defendant EILEEN C VASCO, is an adult individual with last known address of 29 ESSEX RD, CAMP HILL PA 17011. It is averred that Defendant was indebted to CAPITAL ONE BANK N.A. on March 19, 2003 with account number ************0583 (hereafter referred to as "Account"). A copy of the account history is attached here to and collectively marked as Exhibit "A." 4.
