the Court finds, upon clear and convincing evidence, that Jeffrey David Sweet is an
incapacitated person in that he suffers from a mental impairment that prevents him from
receiving and evaluating information effectively and prevents him from formulating and
communicating decisions to such a significant extent that he is unable to manage his financial
affairs on a daily basis.

The Court appoints Maranatha-Carlisle as Permanent Plenary Guardians of the Estate
of Jeffrey David Sweet.
Cumberland Maranatha-Carlisle shall be entitled to charge a fee pursuant to the
Medical Assistance regulations and if there are insufficient assets to pay said fee, Maranath -
