1. Claimant, 84 Lumber Company, L.P., 1019 Route 519 - Bldg. #2, Eighty Four, PA 15330, Phone: (724) 228-8820, Fax: (877)
341-2753, clailms this lien as a subcontractor.
2. The name and address of the Owner or Reputed Owner is Luther Allen Mountz, III and Pamela Rae Mountz, 47 Nelson Drive,
Carlisle, PA 17015-
3, The date of completion that labor, services, equipment or materials were last furnished for the project was January 10, 2012.
4. The Claimant contracted with Hughes Contracting, Inc., 1787 W. Trindle Rd., Ste. 801, Carlisle, PA 17015. Claimant gave a
Formal Notice Of Intention To File A Claim on April 23, 2012.
5. A description of the labor, services, equipment or materials furnished for the project is as follows: Building materials, related
materials andlor labor
6. The amount Claimant is due is $4,445.93.
7. The work of improvement at the property is described as follows: 47 Nelson Dr., Carlisle, PA 17015 in the County of
Cumberland, APN: 08-10-0630-102..
A legal description of the Property follows: ALL THAT CERTAIN piece or parcel of ground together with the improvements
erected thereon, situate in Dickinson Township, Cumberland County, Pennsylvania, more particularly bounded and described in
accordance with the Final Subdivision Plan of Green Hill as recorded in Plan Book 55, page 17, as follows:
BEGINNING at a point in the northern right-of-way line of Nelson Drive at the southeastern corner of Lot No. 14 on the aforesaid
Final Subdivsion Plan; thence along said Lot No. 14, North 43 degrees 14 minutes 07 seconds East, 266.67 feet to a point at the
northeastern corner of said Lot No. 14 at lands now or formerly of Paul H. Yorlets, et ux; thence along said lands now or formerly
of Paul H. Yorlets, et ux, South 46 degrees 45 minutes 53 seconds East 150.00 feet to a point at the northwestern corner of a
fifty (50) feet wide right-of-way for future access; thence along said fifty (50) feet wide right-of-way for future access, South 43
degrees 14 minutes 07 seconds West, 266.67 feet to a point on the said northern right-of-way for future access; thence along
said northern right-of-way line of Nelson Drive, North 46 degrees 45 minutes 53 seconds West, 150.00 feet to a point in said
northern right-of-way line of Nelson Drive at the southeastern corner of said Lot No. 14, the Place of BEGINNING.
CONTAINING 40,000 square feet and being all of Lot No. 15 on the aforesaid Final Subdivision Plan, and being improved with a
2 story dwelling house known as and numbered 47 Nelson Drive, Carlisle, Pennsylvania 17013.
