CLAIM
To the Clerk of the Orphan's Court Division:
Index and make proper entry in your official records of the claim of JEANETTE
BEAKES, in an unliquidated amount in excess of Ten Thousand and no/100 Dollars
($10,000.00), against the Estate of the above named decedent. This claim is filed under Section
3532(b)(2) of the PEF Code, 20 Pa.C.S. § 3532(b)(2).
The basis for the claim is a Complaint filed against Mandy Angle, Administrator of the
Estate of the above named decedent in the General Court of Justice, Superior Court Division,
Beaufort County, North Carolina, and docketed at 12-CVS-114, and arising out of a motor
vehicle accident involving the decedent while driving in North Carolina.
The said decedent, whose last known residence was at 35 Cleversburg Road,
Shippensburg, Cumberland County, Pennsylvania died on September 24, 2011.
Written notice of this claim is being concurrently to Mandy Angle, 20075 Back Road,
Dolyesburg, Pennsylvania 17219, the Administrator the Estate of the above-named decedent.
