ORDER OF COURT
AND NOW, this 6t" day of ]une, 2012, upon consideration of the °Petition for
Appointment of Guardian for the Estate of Mar r and Leauna Marie Oliver,"
filed by the natural mother of Mary Jane Oliver and Leauna
Marie Oliver ( , Emily M. Pivovarnik, and it appearing that
Petitioner has now filed "Exhibit A" that indicates the values of various accounts of
which the minor children are beneficiaries, the Petition is GRANTED to the extent as
follows:
1. RICHARD L. PIVOVARNIK, the maternal grandfather of the minor
children, is hereby appointed the Guardian of the Estate of the minor
persons Mary Jane Oliver and Leauna Marie Oliver;
2. COUNSEL is hereby authorized and specifically directed to
execute all documentation necessary to deposit the funds belonging to
the minor children in an interest-bearing savings certificate in a
federally insured bank or savings institution having an office in
Cumberland County, to be deposited in the name of the respective
minors only.
3. The accounts created for Mary Jane Oliver and Leauna Marie Oliver
pursuant to this Order shall bear the Express Restriction that there
shall be no withdrawals from each of the minor's account prior to the
respective minor having obtained the age of majority, except (i) for
payment of state or federal income taxes, or (ii) upon subsequent
Order of Court.
Within twenty (20) days from the date of this Decree, counsel for Petitioner
shall file an Affidavit with the Court of Common Pleas certifying compliance with this
Decree.
