Cross Secre(ary ANNUAL GUARDIANSHIP REPORT Terry L Barley FOR WILLIAM JAMES HILL DlYeC~r°r May 31, 2012 Report from Guardian of Person and Estate Cumberland County Aging and Community Services Janet Paull, Aging Care Manager 3 PERSON On April 15, 2010 Judge Ebert appointed the Cumberland County Aging and Community Services Plenary Guardian of Person and Estate for William James Hill. Mr. Hill was a resident of Manor Care Health Services-Carlisle at the time of the hearing and continues to reside there. Mr. Hill had not been receiving adequate from the residence where he had been staying prior to his admission to the nursing home. At the time of the Guardianship, Mr. Hill was diagnosed with severe malnutrition, urinary tract infection, acute renal failure, deconditioning, hypertension, chronic anemia and dementia. Mr. Hill is wheelchair bound but is very good at maneuvering around the halls. He likes to participate in activities such as bingo (where he wins many stuffed animals), guessing games, and singing. Mr. Hill is incontinent of urine and wears a catheter and occasionally has urinary tract infections from it. In November 2011, he was admitted to Holy Spirit Hospital for a severe back pain which turned out to be the result of a severe urinary tract infection. He was treated with antibiotics for 4 days. The main problem has been Bill falling out of bed. He has a remote for his TV but he often misplaces it and tries to reach it from his bed and falls. We purchased a new flat screen TV and remote to try to prevent this. But later he rolled out of bed reaching for his shoes. He also began falling. Around March 2012 Bill started to experience a decline in his health. He is very tired most of the time and has lost his appetite. Bill has been placed on Hospice to give him extra support and to encourage him to eat. Mr. Hill is visited on a monthly basis by Aging staff. During the year, Bill was brought small gifts which were purchased by Aging staff. A1' www.ccpa.net/aging ~~ ESTATE After the guardianship appointment, the agency opened a guardianship checking account at M&T Bank. The agency applied for and became his Social Security representative payee. During 2011 his Social Security was $620 and in 2012 it was raised to $642. His Room and Board at Manor Care varies according to the number of days in the month but was usually around $513.02 in 2011. It increased to between $535 and $557.02 in 2012. His expenses during this time include $62 a month paid to the Cumberland County Aging and Community Services for a guardianship fee as allowed by Medical Assistance and approved by the Guardianship Court Order. The only other expense was $169.59 was for several items of clothing purchased at K-Mart. The balance in his account as of March 2012 was $1447.20. When Mr. Hill's medical condition began to decline we checked with Tri-County Memorial Gardens about his pre-arrangements for his funeral. He had purchased a vault and burial lot. He will still need to purchase a casket and pay for the opening/closing. We are considering the possibility of selling the vault and lot and making arrangements with the VA. It is the request of the Cumberland County Aging and Community Services that the agency retain Guardianship of Person and Estate for William Hill. Manor Care Health Services has demonstrated an adequate quality of care for his medical problems.
