Cumberland County Aging and Community Services was named as the Permanent Plenary Guardian of Person and Estate for Sibyl Arms. PERSON Following the hearing Ms. Arms was returned to the Golden Living-West ShorE~ Center where she had been residing since being removed from her home by Emergency Involuntary Intervention. Her house had been condemned by the Borough of Cramp Hill as unfit for human habitation. She underwent surgery at Hershey Medical Center to remove an old shoulder implant because it had become infected. She ended up having a full shoulder replacement. From Hershey she was transferred to Holy Spirit for gall bladder surgery. From there she was admitted to Claremont Nursing and Rehab Center on January 21, 2011 where she continues to reside. She has been diagnosed with senile dementia alzheimer's type, mild hypertension, generalized weakness, anxiety, depression/bipolar disorder, right had contractures, functional incontinence dysphagia, and dementia. Medications include Synthroid .075 mg, Aspirin 81 mg, Depakiine 250mg, Aricept 5 mg, Vitamin D 1000, Pepcid 20mg. By August 2011 Sibyl had gained 14 pounds since being admitted to Claremont in January. She was placed on a low carb diet. Her bipolar disorder is under coni:rol and she is not experiencing any major highs or lows. Staff has commented that shE~ likes to watch what everyone else is doing. In November the nursing staff removed the roam alert bracelet as she has not made any attempts to elope. She had lost nine pounds since putting her on a low carb diet and her weight is now stable. She has been placed on a mechanical soft diet but she does not seem to mind it. During the year she received Occupational Therapy for a while for exercises and self- care and Speech Therapy for swallowing. On the monthly visits, she appears well, is groomed nicely and seems to be happy. On her 75th birthday in March our staff member provided her with a card, a balloon, and a necklace. Necklaces are one of her favorite things. In the activities rooms, the group from her floor crowned her queen and sand happy birthday. She was very pleased. Since she has been placed on an anti-~ depressant she has become more social and seems to enjoy getting out of her room. She enjoys being fashionable. She feeds self about 50% of the time with the aides assisting 50% of the time by supervising, prompting and at times actually feeding her. Sibyl enjoys activities and really likes Bingo, singing, ball and balloon games, and Bible activities. It is the request of the Cumberland County Aging and Community Services that the agency retain Guardianship of Person for Sibyl Arms. The Claremont Nursing and Rehabilitation center has demonstrated an adequate quality of care for her medical problems. We believe, therefore that Ms. Arms should continue to reside in that nursing facility. ESTATE A guardianship account was opened at Wachovia Bank where Ms. Arms had her accounts. In October 2010, Ms. Arms' house and property were sold for $47,800.00, which was $5,800.00 over the appraised value. .Application for Medical Assistance was made at the time of her admission to Golden Living. It was discontinued at the time of the sale of the house until her assets dropped below $8000.00. Prior to that, we were required to cash in her policy with Bankers Life and Casualty which has a cash surrender value of $237.50. Application was nnade for Medical Assistance which began in May 2011. Sibyl's income includes Social Security which was $981 during 2011 and increased to $1017 in 2012. She also receives a retirement annuity which was $397.12 during 2011 but jumped to $530.05 in 2012, In April 2011 she received a refund from Golden Living for $4258.37 and then $1273.50 in September. In addition to the cost of her room and board at the nursing home, a guardianship fee of $150 a month for 6 months totaling $900 was paid to Cumberland County Aging and Community Services. The Agency's fee schedule was approved by the court. The balance of her account at the end of March 2012 was $8,818.43 Attached is a financial summary of Sibyl Arms' income and expenses.
