receipt requested to Transport Services.
06/25/2012 05:39 PM - Robert Bitner, Deputy Sheriff, who being duly sworn according to law, states that on June 5,
2012 at 1739 hours, he served a true copy of the within Writ of Summons, upon the within named
defendant, to wit: Eber E. Fraker, by making known unto himself personally, at 9 Deer Trail, Newville,
Cumberland County, Pennsylvania 17241 its contents and at the same time handing to him personally th
said true and correct copy of the same.

and inquiry for the within named defendant to wit: Arlene Fraker, but was unable to locate her in his
bailiwick. He therefore returns the within Writ of Summons as not found as to the defendant Arlene
Fraker. Request for service at 493 Doubling Gap Road, Newville, Pennsylvania 17241 the Defendant a;
not found. Deputies were advised Arlene Fraker is deceased.

of Summons upon the within named defendant, Transport Services, in the following manner: On June 14,
2012 the Sheriff mailed by certified mail, return receipt requested a true and correct copy of the within ri,
of Summons to the defendant's last known address of 1710 McDuff Avenue North, Jacksonville, Florid
32254. The certified mail return receipt card was received by the Cumberland County Sheriffs Office
"UNCLAIMED", "UNABLE TO FORWARD" by the United States Postal Service on July 11, 2012.
