3 percent [72 P.S. §9116 (a) (1.1) (i)].
For dates of death on or after Jan. 1, 1995, the tax rate imposed on the net value of transfers to or for the use of the surviving spouse is 0 percent
[72 P.S. §9116 (a) (1.1) (ii)j. The statute does not exempt a transfer to a surviving spouse from tax, and the statutory requirements for disclosure of assets and
filing a tax return are still applicable even if the surviving spouse is the only beneficiary.
For dates of death on or after July 1, 2000:
• The tax rate imposed on the net value of transfers from a deceased child 21 years of age or younger at death to or for the use of a natural parent, an
adoptive parent or a stepparent of the child is 0 percent [72 P.S. §9116(a)(1.2)].
• The tax rate imposed on the net value of transfers to or for the use of the decedent's lineal beneficiaries is 4.5 percent, except as noted in
72 P.S. §9116(1.2) [72 P.S. §9116(a)(1)].
• The tax rate imposed on the net value of transfers to or for the use of the decedent's siblings is 12 percent [72 P.S. §9116(a)(1.3)]. Asibling is defined, under
Section 9102, as an individual who has at least one parent in common with the decedent, whether by blood or adoption.

It is my intention by this Will to dispose of all the properly which 1 may own.

It is my desire that a funeral and burial consistent ~.vith my economic status in life. be
conducted as soon as may be practicable following my death and that my remains be buried at
the SS. Peter and Paul Orthodox Church Cemetery in Windber, Pennsylvania, in the 13alog
1~u~1i1y plot. In n1y distribuliotl, I have included ~~ percentage bequest to the church for perpetual
nl~iintenance of the Balog gave site. It is my desire shat my personal representatives ensure that

any special needs to my family headstones and plot, including, but clot limited to, repair of
sinking, erosion, topsoil replacement and the like, shall be performed as a continuing condition
to the acceptance of the bequest by the church. I further direct my personal representatives to
contact the William Kisiel Funeral Home presently located at 1015 Graham Avenue, Windber,
Pennsylvania, as 1 have previously made arrangements with that funeral biome for my burial.
