O'Brien, Esquire, and request that KenCrest Services, 502 West Germantown Pike, Suite 201, Plymouth Meeting PA 19462, by its CEO, currently James McFalls, and his successors be substituted as the Guardian to the plenary guardianship of GARY GRAHAM, currently managed by MH/IDD, and, in support thereof, sets forth the following: The Administrator's office of the County MH/IDD had been serving as a guardian of GARY GRAHAM since 1986. 2. GARY GRAHAM is an adult individual living in an adult group home at KenCrest Group Home, 1001 Thousand Acre Road, Sellersville, PA. 3. On or about the 5`h day of December, 1986, this Court adjudicated GARY GRAHAM wholly incapacitated as a result of his mental retardation resulting from his having an intelligence quotient in the profound range and appointed MHMR as his plenary guardian. 4. The County Administrators have been performing the duties of guardian. for the past years. Petitioner feels that the appointment of KenCrest Services as Guardian would guarantee continuity in the care of GARY GRAHAM as it is expected he will continue to reside in the group home for many years. 10. GARY GRAHAM has no children or grandchildren. i ., 11. GARY GRAHAM's parents are both deceased. 12. GARY GRAHAM receives services through Cumberland County's Mental Health IDD program. 14. The proposed Guardian, KenCrest Services by its CEO, has no interest adverse to the interest. of GARY GRAHAM. 50 Pa. C.S.A. Section 5514, provides that the Court may appoint a successor guardian without the requirement of a hearing. 1 S. Petitioner requests the appointment of KenCrest Services, by its CEO, as Plenary Guardian of the Person and Estate of Mr. Graham. 15. The consent of the proposed Guardian is attached hereto. WHEREFORE, the Petitioner respectfully requests that this Court appoint KenCrest Services by its CEO, currently James McFalls, and his successors as Plenary Guardian of the Person and Estate of GARY GRAHAM. I am 18 years of age or older; 2. I am Chief Executive Officer of KenCrest Services. 3. KenCrest Services has bee providing the full and complete care for Gary Graham since October 1, 2003 at a group home in Sellersville, Pennsylvania; 4. I anticipate that KenCrest will continue, for the foreseeable future, continue to provide said care; 5. I am a citizen of the United States; 6. I do not have any interest adverse to the interests of GARY GRAHAM; and 7. He requires 24 hour supervision and support. Gary utilizes a wheelchair. Staff provide total hands on assistance to Mr. Graham to meet all his everyday needs such as eating,dressing, bathing,etc.... Mr.Graham is non-verbal and communicates by smiling,crying,and using moans or laughs. V. GUARDIAN'S OPINION A.
