MUNICIPAL LIEN FOR SERVICES AND SEWER RATES
AND NOW, comes Monroe Township Municipal Authority, of 1220 Boiling Springs
Road, Mechanicsburg, Pennsylvania, by and through its Solicitor, Michael J. Pykosh, Esquire,
and files the following municipal claim against David J. Monoski and Sherry J. Monoski,
husband and wife, owning property located at 1507 York Road, Carlisle, Pennsylvania 17015.
David J. Monoski and Sherry J. Monoski are the owners of all that certain lot of ground,
including any improvements thereon, being more fully described as follows:
ALL THAT CERTAIN tract of land with the improvements thereon
erected, situate in Monroe Township, Cumberland County, Pennsylvania,
bounded and described as follows:
BEGINNING at a point on the Southern side of Pennsylvania Highway
No. 74, on the dividing line between Lots Nos. 3 and 4 on the hereinafter
mentioned Plan of Lots; thence by said dividing line, South 32 degrees 44
minutes 55 seconds West, 200 feet to a point; thence by the dividing line between
Lots Nos. 4 and 6 on said Plan of Lots, North 57 degrees 15 minutes 05 seconds
West, 100 feet to a point; thence by the dividing line between Lots No. 4 and 5
on said Plan of Lots, North 32 degrees 44 minutes 55 seconds East, 200 feet to a
point on the Southern side of Pennsylvania Highway Route No. 74 aforesaid;
thence by the Southern side of said Highway, South 57 degrees 15 minutes 05
seconds East, 100 feet to the place of BEGINNING.
BEING Lot No. 4 on the Plan of Lots of Raymond E. Diehl and
Genevieve A. Diehl, husband and wife, as recorded in the Office of the Recorder
of Deeds for Cumberland County in Plan Book 27, Page 115; and being subject to
the building setback lines as thereon shown.
BEING improved with a ranch type dwelling house.

BEING THE SAME PREMISES which Pamela J. Williams, formerly
Pamela J. Morrow, and Thomas D. Williams, husband and wife, by Deed dated
May 26, 1995 and recorded May 26, 1995 in the Recorder of Deeds Office in
Cumberland County, Pennsylvania, in Book 122, Page 778, granted and conveyed
unto David J. Monoski and Sherry J. Monoski, husband and wife.
TAX PARCEL NO. 22-11-0282-003E.
Monroe Township Municipal Authority hereby files this Municipal Lien for Services and
Sewer Rates levied and assessed from through and including. Said Authority services and sewer
rates were levied and assessed pursuant to Authority Resolution No. 02-04, same being known as
a Resolution of the Board of Monroe Township Municipality Authority imposing user charges to
be collected from the owner of each improved property served or to be served by the Authority's
sewer system, in conjunction with all prior relevant Resolutions and Ordinances, including, but
not necessarily limited to, Monroe Township Ordinance No. 3 of 1978 and Monroe Township
Ordinance No. 99-6, as amended; and providing for payment and collection of such user charges.
Attached hereto and incorporated herein are the statements forwarded by Monroe Township
Municipal Authority to for sewerage due to be treated from January 1, 2012 through March 31,
2012, which services total is in the amount of five hundred sixty six 501100 ($566.60) dollars,
which sum was duly assessed against, and the real property described herein from the date of
said billing statements as attached hereto, and for which sum, with interest in the amount of ten
(10%) percent, along with penalties and all costs, including attorneys' fees, a municipal lien is
claimed against the above-described real property and premises in accordance with the Act of
May 16, 1923, P.L. 207, §7, as amended, 53 P.S. §7101, et seq, as amended (Municipal Claims
and Tax Liens).

Dear Mr. Monoski:
I am contacting you on behalf of Monroe Township relative to your sewer account
#1341 for real estate owned by you, same being known and numbered as 1507 York
Road. This account is delinquent. As of the last billing notice, same being dated
February 15, 2012, the delinquency is in the amount of $566.50. The purpose of this
letter is to give you the following three options regarding your past due account:
A. Full Payment of the entire outstanding balance to be made within
ten (10) days of the date of this letter;
B. Contact the Authority to set up a Payment Agreement of a minimum
of $100.00 per month on account of the arrears and required to
keep up with all quarterly charges billed thereafter. The Agreement
must be finalized within ten (10) days of the date of this letter;
C. The lien/sheriffs sale process will immediately commence if there is
no communication from the owner to set up and complete either
option A or B within ten (10) days of the date of this letter.
Option C will result in filing fees, service costs, and attorney's fees. Attorney's
fees that are incurred with respect to the collection of this delinquent account will be
assessed at the current rate. The imposition of assessment of attorney's fees may be
avoided by you making payment in full or executing a Payment Agreement within ten
(10) days of date of this letter.
