Prior to this appointment Mrs. Ceja resided in her own apartment at 1108 Y d ver on Drive Apt. C4 Camp Hill, Pennsylvania. She was admitted to the Forest Park Health Center Carlisle on May 27, 2005 after the outcome of an Em ergency Guardianship hearing. Mrs. Ceja continues to reside at Forest Park Health Center. She was moved from the dementia unit to the nursing unit in Au ust 2007 D g . ue to her severe cognitive loss she was no longer benefiting from the activities and structure of the dementia unit She . continues to be diagnosed with senile dementia Alzheimer's type, depression, Parkinson's, PVD, osteoporosis, hypertension and anxiety. Her medical and cognitive t s atus have both declined. She is now nonverbal as well as non-ambulatory. The staff at Forest Park Health Center provides 24 hour su i i perv s on, maximum hands on assistance with activities of daily living, two assists for all transfers, monitoring of skin as she is prone for breakdown and providing necessary verbal cues and redirection. Mrs. Ceja sees her doctor on a routine basis. Mrs. Ceja no longer sees the psychiatrist a h h s s e as been emotionally stable and has not displayed behavioral problems. She is taken to activities, including Mass, but is not able to participate. Mrs. Ceja is no longer on a regular diet but is on a pureed diet with thin liquids. She eats about 50-75% of her meats and requires total assistance at mealtime. She receives fortified milk and an ice cream supplement. Her weight is constant at 143.8 pounds. When visited she occasionally opens her eyes but keeps them shut most of the time. She receives restorative nursing which includes range of motion exercises once and day and hand exercises to increase flexibility in her fingers. She is currently wearing bilateral upper extremity elbow splints to prevent contractures. In addition to visitin client, care plans are attended every 90 days and the medical record is reviewed and discussed with the nursing staff. The staff at the nursing home provides updates and notifications of any changes that Theresa may have experience. It is the request of the Cumberland County Office of Aging that the Agency retains Guardianship of Person and Estate of Theresa Ceja. Forest Park Health Center has demonstrated an adequate quality of care for client by managing her dail needs a continuously monitoring her medical conditions. Mrs. Ceja's appearance s always good and she appears to be content in her present living situation. Therefore, we believe that Mrs. Ce~a should continue to reside at Forest Park Health Center. ESTATE Funeral arrangements were made with Hollinger Funeral Home in Mt. Holly. Mrs. Ce'a has an irrevocable burial trust with Sovereign bank. Payments were made to Forest, Park health Center for .her room and board as directed by Medical Assistance. The Office of Aging also received $100 a month for Guardianship fee as provided for in the guardianship court order. Christmas gifts were donated Home Instead, a home health agency. Her currently income includes $225.44 from a state annuity, $156 state sivit service and $699 from Social Security. Her income tax information was take to AARP to be completed. Her interest dropped below the amount required to file so we no lonter have to file her taxes.
