On May 1, 2009, Temporary Guardianship was granted to the Office of Ag~rig. She o ~_ ~
placed in Claremont Nursing and Rehabilitation .Center where she adjusted quickly t ~
her new surroundings.
On May 26, 2009 the Office of Aging was granted Permanent Guardian of Person.
Ms. Chronister has had ambulation problems. In July 2011, she was given physical
therapy for 3 times a day for 30 days to increase strength, balance, endurance and
mobility. The problems are secondary to arthritis and gait. In July she had surgery to
remove 2 growths, one on each side between her eye and ear. They were not only
growing on the outside, but are also growing on the inside of her skin. Surgery took
place at Carlisle Regional Hospital.
In October, 2011 Ms. Chronister received a letter from her daughter who is currently
serving a prison sentence in Munch, PA. It spoke of her father's death and satanic
rituals that she endued while growing up. The letter was not given to Winnie but is kept
in her file in our office.
During the day, she is usually in a recliner and likes to hold her stuffed animals and a
baby. She likes to socialize and attends most activities although does not directly
participate in them.
Ms. Chronister is diagnosed with chronic obstructive pulmonary disease, anemia,
dementia, coronary artery disease, Depression, peripheral vascular disease, arthritis,
ambulatory dysfunction, GERD, congestive heart failure, and arteriosclerotic disease.
Her weight is constant at 137.1 and her health remains stable. She is currently on a
Functioning Maintenance Program. The nursing staff walks with her to and from the
bathroom and pertorm range of motion exercised on her lower extremities.
Ms. Chronister was visited by staff every 30 - 60 days. Staff also attends quarterly care
plan meetings at the Home.

It is the request of the Cumberland County Aging and Community Services that
the agency retain Guardianship of Person for Ms. Chronister. The Claremont Nursing
and Rehabilitation Center has demonstrated an adequate quality of care for her medical
problems. We believe, therefore, that Ms. Chronister should continue to reside in that
nursing facility.

Ms. Chronister has a guardianship account at Wells Fargo Bank. Ms. Chronister's
monthly income from Social Security is currently $967.00. The amount she receives
increased from $345 due to the Social Security entitlement she receives from the death
of her third ex-husband. Ms. Chronister is on Medical Assistance and payments to the
home consist of her income less insurance premiums of $7.80 a month, $100 monthly
guardianship fees to Cumberland County Aging, as allowed in the court order. She is
also allowed to retain $45.00 under MA rules. Attached is a financial summary of
Winifred Chronister's income and expenses.
