PETITION FOR NAME CHANGE
IIAND NOW, comes Daniel Lee Bubb, by and through his Attorney, Jane Adams, Esquire,
and respectfully requests this Honorable Court to change his name for the following reasons:
1. Petitioner, Daniel Lee Bubb, (hereinafter referred to as "Petitioner currently lives ?t
15 Bellaire Avenue, Carlisle, Cumberland County, Pennsylvania, 17013.
2. Petitioner Daniel Lee Bubb is forty one (41) years old. His date of birth is July 28,
1970.
3. Petitioner has lived at 15 Bellaire Avenue , Carlisle, Pennsylvania since 2010.
4. Petitioner was adopted by his step-father and his last name was changed to Bubb wh?n
he was only ten (10) years old.
Petitioner's step-father is deceased and Petitioner now wishes to resume using his
name, which is Daniel Lee McCorkel.
6. Petitioner's brother Donald has also resumed using the last name of McCorkel.
7. Petitioner plans on going into business with his brother and he believes that using
natural Father's last name would avoid confusion.
8. Petitioner has no outstanding credit debt that could be avoided through a name
a(L4C.1.49 9
9. Petitioner has no outstanding federal or state tax debt that could be avoided through
name change.
10. Petitioner previously used the requested name through 1970, when he was ten years
old.
11. Petitioner is requesting that this Honorable Court change his name to: Daniel Lee
McCorkel..
WHEREFORE, Petitioner requests that this Honorable Court Order change his name.

has applied for a name change, has been searched in the files of the Pennsylvania State Police. Criminal Records and
Identification Division Central Repository, and:
( x) This person is not subject to 18 PA C.S. Chapter 91. and the fingerprints have been destroyed.
( ) This person is subject to 18 PA C.S. Chapter 91, and the name change has been noted on the person's
criminal history record information.
( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE NAME
CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN A NAME
CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The Pennsylvania State
Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).

( ) A criminal record was revealed that might prohibit a name change. Because the subsection(s) of the
