TO THE DEFENDANT: The above named plaintiffs asks judgment against you for $ 5824.74 togeth r with

violated:
At the request of the Defendants, the Plaintiff provided various restaurant
equipment and/or supplies. Plaintiff has provided all of the equipment and/or
supplies ordered by Defendant. The prices charged Defendant by Plaintiff
were fair and reasonable and were the prices which Defendant agree to pay.
Despite repeated demands by Plaintiff, the Defendant has refused an
continues to refuse to tender payment to Plaintiff for the outstanding amount
due of $5684.74. Plaintiff demands judgment against the Defendant i the
amount of $5684.74 together with interest and costs of suit.
