AND NOW, July 9, 2012, an appeal having been filed with the Superior Court, it is,
ORDERED AND DIRECTED that the Appellant file a Concise Statement of Matters
Complained of, with respect to the appeal to the Superior Court filed in this matter and a
statement identifying any transcript which shall include the hearing dates, which may be
necessary for appellate purposes, before this Court files its Final Memorandum in this case. A
copy of said statement shall be forwarded to the Court Administrator.
Said Statement must be filed within twenty-five (25) days from the date of this Order, and
served pursuant to Pa.R.A.P. 1925(b)(1). Furthermore, any issue not properly included in the
Statement timely filed and served pursuant to Pa.R.A.P. 1925(b) shall be deemed waived.
