The Petition of Robert L. Foltz, Jr., by his attorney, Wayne F. Shadeuire, ,~,
respectfully represents, as follows: °'
1. Petitioner Robert L. Foltz, Jr. is an adult individual who resides at 1900
Douglas Drive, Carlisle, Cumberland County, Pennsylvania 17013.
2. Petitioner is the Executor of the Last Will and Testament and Estate of his
mother, Madeline G. Foltz.
3. The decedent died on Apri124, 2011.
4. Article I of the Last Will and Testament gave the residue of the Estate to her
granddaughter and the daughter of Petitioner, Marcy J. Foltz, born June 7, 2004.
5. The approximate balance of the residue of the Estate is Thirty Thousand and
No/100 ($30,000.00) Dollars.
6. The minor beneficiary resides with Petitioner and with her mother, Kimberly
M. Foltz, at the aforesaid address of Petitioner.
7. Petitioner believes and therefore avers that the interests of the minor
beneficiary will be fully and adequately protected by deposit of the funds into a
federally insured, sequestered account in the name of Marcy J. Foltz, a minor, at Farmers

and Merchants Trust Company, 14 North Hanover Street, Carlisle, Pennsylvania 17013,
with the restriction that no withdrawals from the account will be permitted until the minor
attains the age of eighteen (18) years, except as authorized by prior Order of Court.
WHEREFORE, Petitioner respectfully requests that your Honorable Court order
that the residue of the Estate be deposited into a federally insured, sequestered account in
the name of Marcy J. Foltz, a minor, at Farmers and Merchants Trust Company, 14 North
Hanover Street, Carlisle, Pennsylvania 17013, with the restriction that no withdrawals
from the account will be permitted until the minor attains the age of eighteen (18) years,
except as authorized by prior Order of Court.
