Reason For This Request
Personal Statement
I, Angie Strevella, request a name change for my minor child, Elijah Alexander Florence. I am
petitioning that Elijah's name be legally changed to Elijah Alessandro Strevella. Prior to getting married
I raised Elijah alone. Elijah's biologically father has never been involved in his life. The absent parent as
only seen Elijah twice since his birth (at 4 months of age and again when Elijah was 2 years old). The
absent parent has never attended any of Elijah's doctor appointments, birthdays, or school events. T e
absent parent has never visited Elijah during holidays or any other special occasion. The absent parent
has never made a legitimate effort to be in Elijah's life. Moreover, although a DNA test was performed
to establish paternity, the absent parent is not named on Elijah's birth certificate nor has he ever lega ly
requested a modification of Elijah's birth certificate to show that he is Elijah's biological father. I don t
know the absent parents whereabouts nor do I know where the absent parent resides.
Gennaro Strevella, my husband, has been the only father figure that Elijah has known. Elijah
refers to Gennaro as his father. Elijah, although very young, also desires having his name changed to
Eliiah Alessandro Strevella. He stated "1 want to feel like part of the family". In the future Gennaro an
will arrange for Elijah to be legally adopted by Gennaro. For the time being I am seeking a name chant
I believe that Elijah does not have a strong connection to his surname. While Elijah is very much loved
home, I fear that he will suffer feelings of "not belonging" due to him having a different surname.
Gennaro and I would like the name change to take place before the beginning of the 2012-2013 schoc
year so Elijah does not suffer any embarrassment and mockery at the hands of his peers for having
dissimilar surname than Gennaro and I.
