Dear Ms. ERIN FERGUSON:
We have received medical information indicating that you have a Seizure Disorder condition
which prevents you from safely operating a motor vehicle.
As of 08/28/2001, you may no longer drive. Your driving privilege is hereby recalled
indefinitely as mandated by Section 1519(c) of the Vehicle Code.
This decision has been made by comparing your medical condition with the standards
recommended by our Medical Advisory Board and adopted by this Department. This action will
remain in effect until we receive medical information that your condition has improved and you
are able to safely operate a motor vehicle.
Since you have a seizure disorder, you must remain seizure free for a six month period, with or
without medication, before you will be eligible for reinstatement of your driving privilege. Our
records indicate that your seizure occurred on 7/4/01.
In order to comply with this action, you must return all current Pennsylvania driver's licenses,
learner's pernuts, temporary driver s licenses (camera cards) in your possession, on or before the
effective date listed above. If you cannot comply with the requirements stated above, a sworn
affidavit stating that you are aware of the sanction against your driving privi!ege must be
submitted. When the Department receives your license or affidavit, we will send you a receipt.

PURPOSES. However, you may apply for and obtain a photo identification card at any Driver
License Center. You must present two (2) forms of proper identification (e.g., birth certificate,
valid U.S; passport, man-iage certificate, etc.) in order to obtain your photo identification card.
You have the right to appeal to the Court of Common Pleas (Civil Division) within thirty (30)
days of the mail date of this notice. If you file an appeal in the County Court, the Court will give
you a time-stamped certified copy of the appeal. In order for your appeal to be valid, you must
send this time-stamped certified copy of the appeal by certified mail to:

Me.in is a nonstcroidal anti-infla~ato~ drag available in both prcsaription ~d
nonprcscription fo~s. Prescription Motrin is used in adults for relief of thc s~ptoms of
rheumatoid mhritis ~d ostana~hritis, ~ea~ent of menstrual pain, and relief of mild to
moderate pain. In children aged 6 months ~d older it can be given to reduce fever ~d
relieve mild to moderate pain. It is also used to relieve thc symptoms of juvenile a~ritis.
MoWin lB tablets, caplets, and gclcaps; Children's Motrin Suspension; ~d Advil tablets and
caplets are available without a prescription. Check the packages for uses, dosage, ~d other

Most impo~ant fact about this drug
You should have frequent checkups with your doctor if you take Motrin regularly. Ulcers or
internal bleeding can occur without warning.
How should you take this medication?
Your doctor may ask you to take Motrin with food or an antacid to avoid stomach upset. The
suspension can be given with meals or milk if it upsets the stomach.
A drink of water or other fluid after taking a chewable tablet can help your body absorb the
drug.
If you are using Motrin for mthritis, you should take it regularly, exactly as prescribed.
-If you miss a dose...
Take it as soon as you remember. If it is almost time for your next dose, skip the one you
missed and go back to your regular schedule. Never take 2 doses at the same time.

What side effects may occur?
Side effects cannot be anticipated. If any develop or change in intensity, inform your doctor
as soon as possible. Only your doctor can determine if it is safe for you to continue taking
Motrin.
More common side effects may include:
Abdominal cramps or pain, abdominal discomfort, bloating and gas, constipation,
diarrhea, dizziness, fluid retention and swelling, headache, heartburn, indigestion,
itching, loss of appetite, nausea, nervousness, rash, ringing in ears, stomach pain,
vomiting
· Less common or rare side effects may include:
Abdominal bleeding, anemia, black stool, blood in urine, blurred vision, changes in
heartbeat, chills, confusion, congestive heart failure, depression, dry eyes and mouth,
emotional volatility, fever, hair loss, hearing loss, hepatitis, high or Iow blood
pressure, hives, inability to sleep, inflammation of nose, inflammation of the
pancreas or stomach, kidney or liver failure, severe allergic reactions, shortness of
breath, skin eruptions or peeling, sleepiness, stomach or upper intestinal ulcer, ulcer
of gums, vision loss, vomiting blood, wheezing, yellow eyes and skin
Why should this drug not be prescribed?
If you are sensitive to or have ever had an allergic reaction to ibuprofen, aspirin, or similar
drugs, such as Aleve and Naprosyn, or if you have had asthma attacks caused by aspirin or
other drugs of this type, or if you have angioedema, a condition whose symptoms are skin
eruptions, you should not take this medication.
Make sure that your doctor s aware of any drug react ons that you have experienced.
Special warnings about this medication
Peptic ulcers and bleeding can occur without warning. Tell your doctor if you have bleeding
or any other problems.
This drug should be used with caution if you have kidney or liver disease, or are severely
dehydrated; it can cause liver or kidney inflammation or other problems in some people.
Do not take aspirin or any other anti-inflammatory medications while taking Motrin unless
your doctor tells you to do so.
If you have a severe allergic reaction, seek medical help immediately.
Motrin may cause vision problems. If you experience any changes in your vision, inform
your doctor.
Motrin may prolong bleeding time. If you are taking blood-thinning medication, this drug
should be taken with caution.
This drue can cause water retention. It should be used with caution if you have hJeh blood

pressure~or poor heart function.
Avoid the use of alcohol while taking this medication.
Motrin may mask the usual signs of infection or other diseases. Use with care in the presence
of an existing infection.
If you have diabetes, remember that the suspension contains 1.5 grams of sucrose and 8
calories per teaspoonful.
Motrin chewable tablets contain phenylalanine. If you have a hereditary disease called
phenylk~tonuria, you should be aware of this. -
Possible food and drug interactions when
taking this medication
If Motrin is taken with certain other drugs, the effects of either could be increased, decreased,
or altered. It is especially important to check with your doctor before combining Motrin with
the following:
Aspirin
Blood pressure medications known as ACE inhibitors, including Vasotec and Capoten
Blood-thinning drugs such as Coumadin
Diuretics such as Lasix and HydroDIURIL
Lithium (Lithonate)

Methotrexate (Rheum
Special information if you are pregnant or
breastfeeding
The effects of ibuprofen during pregnancy have not been adequately studied. If you are
pregnant or plan to become pregnant, inform your doctor immediately, lbuprofen may appear
in breast milk and could affect a nursing infant. If this medication is essential to your health,
your doctor may advise you to discontinue breastfeeding until your treatment with this
medication is finished ~
Recommended dosage
ADULT~ - --
Rheumatoid Arthritis and Osteoarthritis
The usual dosage is 1,200 to 3,200 milligrams per day divided into 3 or 4 doses. Your doctor
will tailor the dose to your individual needs. Symptoms should be reduced within 2 weeks.
Daily dosage should not be greater than 3,200 milligrams.
Mild to Moderate Pain
The usual dose is 400 milligrams every 4 to 6 hours as necessary.
Menstrual Pain
The usual dose is 400 milligrams every 4 hours as necessary. Begin treatment when
symptoms first appear.

CHILDREN 6 MONTHS TO 12 YEARS OF AGE
The recommended dose is 5 milligrams per 2.2 pounds of body weight if temperature is less
than 102.5°F or 10 milligrams per 2.2 pounds of body weight if temperature ~s 102.5 F o
greater. The fever should go down for 6 to 8 hours. Do not give the child more than 40
milligrams per 2.2 pounds of body weight in one day.
Mild to Moderate Pain
The usual dose is 10 milligrams per 2.2 pounds of body weight every 6 to 8 hours. Do not
give the child more than 4 such doses per day.
Juvenile Arthritis
The usual dose is 30 to 40 milligrams daily per 2.2 pounds of body weight, divided into 3 or
4 doses Some ch ldren may need only 20 milligrams dai y per 2 2 pounds. ~
Overdosage
Any medication taken in excess can have serious consequences. An overdose of Motrin can
be fatal. If you suspect an overdose, seek medical attention immediately.

Abdominal pain, breathing difficulties, coma, drowsiness headache, irregul~'
heartbeat, kidney failure, Iow blood pressure, nausea, ringing in the ears, seizures,
sluggishness, vomiting
