(Attach proof of insurance to this Appeal and either a notarized statement of PennDOT form MV-221 to document non-operation of the vehicle). ?? Other (specify reason:) Son Patrick, who had Power of Attorney at the time that policy lapsed, did not fulfill his obligation of ensuring that premiums were paid in a timely fashion. Moreover, he did not inform Patricia that premiums were not paid or that policy had lapsed. His Power of Attorney has since been repealed. All mail from insurance company was mailed Patrick, who lives at separate residence from Patricia. Therefore Patricia would not have had any way to be infori of policy's lapsed status.
