AND NOW, comes Petitioner by and through her attorney, Thomas P. Gleason,
and in support of the within petition avers as follows:
1. The Petitioner is Milisa Mae Bowen who was married to William A.
Bowen on November 12, 1993, in Hagerstown, Maryland.
2. The Petitioner's maiden name was Milisa Mae Lehman: a copy of her
birth certificate is attached hereto as "Exhibit A".
3. Petitioner's date of birth is November 12, 1971.
4. Since March 25, 2004, Petitioner has resided at 23 Clover Hill Road,
Newburg, Cumberland County, PA 17240.
5. It is Petitioner's intention to retake her maiden name of Milisa Mae
Lehman as she and her husband attempt to work through some ongoing marital
difficulties and strife.

6. The Petitioner has no judgments, decrees of record or other similar matters
filed against her.
7. The Petitioner has no criminal record and is submitting a completed
fingerprint card to the Prothonotary contemporaneously with the filing of this petition.
WHEREFORE, Petitioner requests that her name be changed to Milisa Mae
Lehman.
