Heckard and Jamie C. Heckard f/k/a Jamie C. Derr (collecti~ referred to as "Petitioners"), by their undersigned attorney, requests that this Court order a of the minor Petitioner's name pursuant to 54 Pa. Cons. Petitioners are adult individuals residing at 207 West Pine Street, Mount Hilly Springs, Cumberland County, Pennsylvania 17065. 2. Petitioners are the natural parents of the Minor Petitioner, Greyson Andrew De 3. The Minor Petitioner was born out of wedlock on March 30, 2007. 4. At the time of Minor Petitioner's birth Jamie C. Heckard was known as Jamie Derr. 5. The Minor Petitioner took his mother's maiden name, "Derr" as his last name. 6. On May 29, 2010, Petitioners were married and Petitioner Jamie C. Heckerd took last name of Petitioner Kurtis L. Heckard. 7. Petitioners reside together with Minor Petitioner. 8. From. his birth until he was eighteen months old Minor Petitioner resided in 21 Dover Bridge Road, Preston, M.D. 21655.
