TO THE! HONORABLE JUDGES OF SAID COURT:
The Petition of Elliott Edward Bricker represents:
1 Petitioner is Elliott Edward Bricker, who resides at 334 Fifth Street, New
Cumberland, PA 17070.
2',. Petitioner's year of birth is: December 11, 1993.

3. Petitioner has resided at the following addresses for the five (5) years previous to
the filing of this Petition: 334 Fifth Street, New Cumberland, PA 17070.
4. Petitioner desires and intends to change his name to: Elliott Edward Keys.
5. Petitioner provides the following reasons for this request: Petitioner's last name
is the maiden name of his mother. When he was born his mother and father were not married.
They were later married and had two (2) other children together who both have the last name of
their father, Tracy Keys. Petitioner would like to have the same last name as his father and his
brothers;
6. The proposed change in Petitioner's name, if granted, will not be detrimental to
the interests of any other person and is not against public interest.

7. The Petition is not filed to defraud creditors.
WHEREFORE, Petitioner requests the Court, pursuant to the Act of December 16, 1982,
P.L. 1309, No. 295 § 701, for the following:
a. to fix a hearing on this Petition;
b. to direct that notice be given on this Petition and hearing; and
c. to enter a decree changing the Petitioner's name from Elliott Edward
Bricker to Elliott Edward Keys.
