MATTHEW BRINK ~ ~; ; - c~ , , -; AN INCAPACITATED ADULT r- ` • ~ N • No. 2009-1198 z c~'' ~"' = _- . `~ ©~- -o , : .=~ , V A~ IN RE: PETITION TO AMEND THE FEBRUARY 26, 2010 ORDER ~ THE C~E1 r% RT'~~' ~ OF COMMON PLEAS OF CUMBERLAND COUNTY , PENNSYLVANIA, ORPHANS COURT DIVISION, DOCKET NUMBER 2009-1198 ORDER OF COURT AND NOW, this 23~day of _ I~-~,.t,~„ ~ , 2012, upon consideration of the Petition to Amend the February 26, 2010 Order of the Court of Common Pleas of Cumberland County, Pennsylvania, Orphans Court Division, Docket Number 2009-1198; IT IS HERBY ORDERED AND ADJUDGED, that Orrstown Bank, (the "Trustee") having been duly appointed guardian of the Financial Estate of Matthew Brink, (the "Beneficiary"), charged with the oversight and manage of his financial and property management affairs, for his benefit, pursuant to the said February 26, 2010 Order, is appointed Trustee of the funds held in Trust at Orrstown Bank, for the benefit of the said Beneficiary, to serve in that capacity, without bond. IT IS FURTHER ORDERED that Trustee shall have the responsibility and fiduciary duty to oversee and manage the funds held in trust for the benefit of the Beneficiary and shall act with the utmost good faith in dealing with the Trust Estate, and shall, in all things faithfully discharge the duties of a trustee as imposed herein; and ~h IT IS FURTHER ORDERED, that the trust estate be used and payments made wholly within the discretion of the Trustees, as to the type and nature of expenditures and the amounts and frequency of payments, for the care, maintenance, support and wellbeing of the Beneficiary. IT IS FURTHER ORDERED, that the Trustee pay the legal fee due and payable to Attorney, Steven R. Snyder, in the amount of $625.00, payable to Rominger & Associates; IT IS FURTHER ORDERED that upon the death of the Beneficiary, the remaining funds held in trust shall be distributed to the Beneficiary's heirs and assigns.
