Gregory I. Knight, Esquire, and represents as follows. -

1. That Mother is Amber Lynn Smith, an adult individual, who resides at 163
Condoguinet Mobile Estates, Newville, Pennsylvania 17241.
2. That Mother is an adult individual over the age of 18.
3. That Mother has been a resident of Pennsylvania since birth.
4. That your Petitioner, Nevaeh Lynn Keebaugh, was born on June 4, 2008 and her
father is Christopher Keebaugh.
5. That Mother and Father have been never married.
6. That Mother desires to change the last name of her daughter to Smith, which is
her Mother's last name, as Father has not seen his daughter in more then 3 years
and has not paid child support since August 2010.
7. That Petitioner's counsel has spoken with Father who told Petitioner's counsel
that he does not object to this Petition.
That the granting of this Petition is in the minor child's best interests.
WHE FORE, pursuant to 54 Pa. C.S. Section 702, your Petitioner requests that the Court
Order that Petitioner's full name shall be Nevaeh Lynn Smith.
