The following represents Mellon Certified Restoration's statement of claim. 1. The name of the Claimant is Mellon Certified Restoration, a Pennsylvania corporation with a place of business located at 5005 Devonshire Road, Harrisbur , Pennsylvania. Mellon Certified Restoration is filing this claim as a contractor in connection wi emergency repairs and subsequent improvements and alterations to the real property set forth herein. 2. The name and address of the Owner or Reputed Owner at the time Mellon Certified Restoration furnished labor, services, goods and materials at the subject property is Ti - to f. 5~~ d G~~ /'7 ,2.~ ~ i f Sey Properties, Inc., a Pennsylvania corporation, with an address located at 48-50 West Main Street Mechanicsburg, PA 17055. ~ 3. The property against which this Mechanics' Lien Claim is filed is the block of ground and all improvements thereon known as 48-50 West Main Street, Mechanicsburg, Cumberland County, Pennsylvania (hereinafter, the "Property"). The county parcel ID number s 19-23-0565-023. 4. Mellon Certified Restoration entered into a Commercial Work Authorization/Contract ("Contract") with Seymour Levine who, upon information and belief, i a shareholder of Tif--Sey Properties, Inc., dated February 28, 2011 wherein Claimant agreed to provide emergency repairs and improvements at the Property as a result of a fire. A copy of the Contract is attached hereto and incorporated herein as Exhibit "A". 5. Claimant provided repairs and improvements at the Property until it's employee were requested to stop working at the Property on March 21, 2012 and Claimant was not allow d to finish the work that was originally agreed upon by the parties. 6. At the time Claimant's services were terminated, it had performed repairs and mitigation due to the fire in the amount of $871,025.15. The nature and character of the labor d materials furnished at the Property are more specifically described in a detailed, itemized breakdown of work prepared by Claimant which is attached hereto and incorporated herein as Exhibit "B". 7. Claimant has been paid $805,000.00 to date for work performed at the Property. 8. The amount due and owing from the Owner to Mellon Certified Restoration for services and materials it provided at the Property is $70,076.69 plus accruing interest. See Exhibit "C". 2 i 1 9. Mellon Certified Restoration completed all work for which it now seeks payment in a good and workmanlike manner. 10. Mellon Certified Restoration last did work on the Project on Mazch 21, 2012. 11. By filing this claim, Mellon Certified Restoration does not waive or intend to waive, modify, impair or postpone its right to assert any other claim which it may have pursu to its performance of work at the Property. 12. This Mechanics' Lien Claim is filed to ensure payment of contract balances due and owing to Mellon Certified Restoration for work performed and material provided at the subject Property. 13. This Lien is a claim against the fee simple interests of the Owner, Reputed Own r and the Subsequent Owner(s) of the aforesaid premises. WHEREFORE, Mellon Certified Restoration claims to have a lien upon the aforementioned Property located at 48-50 West Main Street, Mechanicsburg, Pennsylvania for the amount of $70,076.69, together with lawful interest thereupon, and requests that the Prothonotary enter the claim upon the judgment index and the mechanics' lien doclaet against Owner, Tif-Sey Properties, Inc.
