("Premier Siding"), by its attorneys, Law Offices of Peter J. Russo, P.C., files this Mechanic's Lien Claim, and, in support thereof, avers as follows: 1. Claimant, Premier Siding and Roofing of PA, LLC. is a limited liability company and existing under the laws of the Commonwealth of Pennsylvania with a registered business address of 152 South Hanover Street, Carlisle, PA 17013. 2. The owners of the property subject to the lien are V. Mark and Terri L. Polinka with a principal address of 829 Fairfield Road, Mechanicsburg, PA 17055. A true and correct copy of the deed to the property subject to the lien is incorporated herein, made a part hereof, and attached hereto as Exhibit A. J~~ O' ~9~~~1 ys73 ~~oQ6~ 3. The property subject to the lien is 829 Fairfield Road, Mechanicsburg, PA 17055, also known as tax parcel no. 22-24-0783-059. 4. Claimant is filing this claim as a contractor under the Mechanic's Lien Law of 1963.49 P.S. § 1101 et seq. 5. The last date on which Claimant completed the work for which this claim is being made was March 1, 2012 which is less than six months from the date of this filing. 6. The amount claimed to be due is $5,222.82. 7. The improvement and property claimed to be subject to the lien is a full roof replacement on the real property located at 829 Fairfield Road, Mechanicsburg, PA 17055 wherein Claimant whereby to furnish all labor and materials needed to complete a full roof replacement. A true and correct copy of the written proposal is attached hereto and incorporated herein as Exhibit B. WHEREFORE, Plaintiff, Premier Siding and Roofing of PA, LLC claims to have a lien upon the premises herein described in the amount of $5,222.82, plus interest, costs of suit and such other and further relief as the Court may deem appropriate. ub LA FILE RUSSO, P.C.
