ORDER
AND NOW, TO WIT, this ~~day of ~ , 2012, upon consideration of the
within Petition for Adjudication of Incapaci y and Appointment of Plenary Guardian of the
Person and Estate, in accordance wi/t~h 20 PA C.S.A. Section 551 1, the Court hereby schedules a
hearing for the ~~ day of !~ G~e(~ , 2012, at /!~'3C~ ~ m Court Room
No. ~ of the Cumberland County Courthouse located at 1 Courthouse Square, Carlisle,

whether Emily Jane Miller should be adjudicated to be an i acn pacitated person and a Plenary
Guardian appointed of her Person and Estate.
IT IS FURTHER ORDERED AND DECREEED that at least twenty (20) days' notice of
said hearing shall be given to the alleged incapacitated person by personal service as required by
law, and to the proposed Guardian(s) by certified mail, return receipt requested, of the within
Petition and Citation; and to all other persons listed in the Petition as nearest living relatives, by
certified mail to the addresses set forth in the Petition, and to any other interested party.

incapacitated person in the above-captioned matter.
