PETITION FOR GUARDIANSHIP OF THE_PERSON OF
1. Your Petitioners are Heriberto and Kelifer Caraballo, adult individuals, man'ied to
each other. They are the maternal uncle and aunt of Eriq Armani Caraballo, the minor
who is the subject of this petition. They reside at 19 Essex Road, Camp Hill, Cumberland
County, Pennsylvania 17011.
2. The subject of this petition is Eriq Armani Caraballo, a minor, who was born on
March 12, 2003, and is presently nine years old. A copy of his birth certificate is attached
hereto, marked Exhibit A and incorporated herein by reference.
3. Your proposed guardians are your Petitioners, Heriberto and Kelifer Caraballo who
reside at the address articulated in paragraph 1 above. Heriberto Caraballo is thirty-two
(32) years of age and Kelifer Caraballo is twenty-eight (28) years of age.
4. Eriq Armani Caraballo, the minorwho is the subject of this petition, is currently living
with your Petitioners (having lived with them since September, 2010) and will continue to
do so for the foreseeable future because of the circumstances articulated below.
5. The mother of the minor is Erica M. Caraballo; she has no permanent address, but
borrows accommodations from friends and relatives in the Camp Hill area and, despite her
itinerant status, consents to the guardianship requested in this petition. Her consent is
attached hereto and is included in the description of consents in paragraph seven below.
6. As the attached birth certificate shows, the identity of the minor's father is unknown.
7. Your Petitioners consent to the proposed guardianship. Consents for all persons are
attached hereto, respectively marked Exhibits B and C and incorporated herein by
reference.
8. The religious affiliation of the minor's mother, Erica M. Caraballo is Christian and the
religious affiliation of your Petitioners is Christian.
g. There has been no prior action for the appointment of a guardian involving the said
Eriq Armani Caraballo. The proposed guardians have no adverse interest to the minor and
the minor has no interest in any estate or assets.
10. It is your Petitioners' desire, and the minor's desire, that guardianship as described
above be decreed so that your Petitioners gain the authority to enroll the minor in school
and perform other necessary functions in the best interests of the minor.

The proposed guardians swear that they will continuously support Eriq Armani
Caraballo without the help of any other adult and that they will assume liability for any and
all personal expenses that Eriq Armani Caraballo may incur.

Heriberto and Kelifer Caraballo wilt also be claiming Eriq Armani Caraballo, a minor,
as a dependent on their Income Tax Returns.
WHEREFORE, your Petitioners respectfully request this Honorable Court to grant
the guardianship herein requested.

CONSENT BY PROPOSED GUARDIANS
HERIBERT AND ELIFER ARABALLO
We, Heriberto and Kelifer Caraballo, do hereby certify that we consent to be
appointed guardians of the person of Eriq Armani Caraballo , a minor.
Additionally, we are both citizens of the United States of America and we both
speak, read and write the English language. We are not a fiduciary or an officer or
employee of a corporate fiduciary of an estate in which Eriq Armani Caraballo has an
interest nor a surety, officer or employee of a corporate surety and that we have no interest
adverse to Eriq Armani Caraballo. We also hereby state that, if the proposed
guardianship is granted, we will support the minor, gartis, not seeking assistance from any
other adult in that regard. Further, we will pay any and all personal expenses that the
minor may incur. Finally, if the guardianship is granted, we will be claiming the minor as
a dependent on our Income Tax Returns while the minor is living with us in our home
pursuant to the proposed guardianship (if the proposed guardianship is granted), for as
long as we are eligible to do so.
We verify that the statements made in this Consent are true and correct. We
understand that false statements herein are made subject to the penalties of 18 Pa.C.S.

I, ERICA M. CARABALLO state the following:
1. I am the natural mother of Eriq Armani Caraballo, the minor captioned above who
was bom on March 12, 2003, in the State of Florida.
2. I am an adult individual.
3. I have read the attached Petition and agree with the facts and conclusions as stated
therein.
4. I join in the request to have Heriberto Caraballo and Kelifer Caraballo appointed as
guardians of the person of my natural son, Eriq Armani Caraballo.
5. I verify that the statements made in this Consent are true and correct. t understand
that false statements herein are made subject to the penalties of 18 Pa.C.S. §4904 relating
to unswom falsification to authorities.
