alleged incapacitated person, the Citation With Notice, the Petition for
Appointment of a Plenary Guardian, and the Order issued by the Court on
September 28, 2012, in the captioned above matter. I further certify that I
explained the contents and terms of the Petition, to the maximum extent possible in
language and terms that Jean B, Deissler, an alleged incapacitated person, is most
likely to understand.
