AND NOW, comes Jill Marie Raynor, by and through her attorneys, BARK
SCHERER LLC, and files the within Petition and, in support thereof, sets forth the
following:
1. Petitioner, Jill Marie Raynor, is an adult individual residing at 11 Dogwood
Drive, Newville, Cumberland County, Pennsylvania 17241.
2. Petitioner was born on December 19, 1968 in Huntington, New York.
3. At the time of birth, Petitioner was given the name Jill Marie Kraus. Kraus
was her father's surname.
4. Petitioner's mother was thereafter divorced from Petitioner's father and
married Peter H. Raynor. Petitioner began using the last name Raynor.
5. At the age of sixteen (16), Petitioner's name was officially changed by
Court Order to Jill Marie Raynor. Petitioner was never adopted by Peter H. Raynor. A
copy of the Court Order is attached hereto and incorporated herein as Exhibit "A." A
copy of the birth certificate evidencing this fact is attached hereto and incorporated
herein as "Exhibit B."
6. Petitioner's mother is deceased.
7. Petitioner has no contact with Peter H. Raynor.
8. The Petitioner is not married.
9. Petitioner is employed by Accenture as a healthcare technology
consultant.
10. Over the past 10 years, Petitioner's residence has been 11 Dogwood
Drive, Newville, Cumberland County, Pennsylvania.
11. There are no judgments or decrees of record against Petitioner.
12. Petitioner now desires to have her name changed to Jill Marie Leahy,
since she has no family connection to her present surname and the Leahy surname is
her mother's maiden name.
WHEREFORE, Petitioner requests your Honorable Court to enter an Order
granting the Petitioner's request to change her name from Jill Marie Raynor to Jill Marie
Leahy.

WHEREAS, Petitioner has requested a name change, and
WHEREAS, Petitioner has, in her Petition for Name Change,
alleged and sworn, by her mother, that:
1. Petitioner's name is Jill Marie Kraus, and she is a
resident of Vienna, Fairfax County, Virginia.
2. Petitioner's parents are Robert Anthony Weston, father,
and Bette Jane Raynor, mother.
3. Petitioner was born on December 19, 1968, in Huntington,
New York.
4. She has no felony record and is not presently incar-
cerated or a probationer with any court.
5. Petitioner's mother was married on September 9, 1965,
to Robert Anthony Kraus, and was divorced from him on or about
January 16, 1973. Mr. Kraus changed his name to Robert Anthony Weston
in 1979.

6. Petitioner has not previously changed her name and re-
quests a name change because she lives with her mother and would
like to have her mother's last name for purposes of convenience.
7. The name change is not requested for an illegal purpose.
8. Petitioner's father consents to the requested name
change.
NOW THEREFORE, in compliance with Petitioner's requests,
it is hereby ordered that Petitioner's name be changed to Jill
Marie Raynor.
