North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owner of the property against which this lien is filed is Kenneth W. Greene, Jr., of
1420 Bradley Drive, Unit F-212, Carlisle, Pennsylvania.
A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 1420 Bradley Drive, Unit F-212, Carlisle, Pennsylvania.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq. ~(

The lien is claimed for unpaid water and sewer user fees against the above-described property in the
amount of $606.48, plus any additional fees incurred thereafter.

Interest is claimed on the above-mentioned fees until paid in full.

WITNESSETH, that the said parties of the first part, for and in consideration of the sum of
Eighty-Five Thousand and 00/100 {$,85,000.00) Dollars, lawful money of the United States of
America unto them well and truly paid by the said party of the second part, at and before the sealing
and delivery of these presents, the receipt whereof is hereby acknowledged, and subject to the
reservations hereinafter set out, have granted, bargained, sold, aliened, enfeoffed, released,
conveyed and confirmed, and by these presents do grant, bargain, sell, alien, enfeoff, release,
convey and confirm unto the said patty of the second part, his successors and assigns,
ALL THAT CERTAIN unit designated as No. F-212, being a unit in Pheasant Run
Estates, a condominium located in North Middleton Township, Cumberland County,
Pennsylvania, which unit is located on the Second Floor, Building V, as designated in the
Declaration of Condominium recorded September 24, 1980, in the Office of the Recorder of
Deeds for Cumberland County in Miscellaneous Book 258, Page 6 and in the Declaration Plans,
as recorded in the Office of the Recorder of Deeds for Cumberland County in Plan Book 38,
Page 112.
TOGETHER with all right, title and interest, being a .0059524% interest in and to the
Common Elements as more fully set forth in the aforesaid Declaration of Condominium and
Declaration of Plans.
UNDER AND SUBJECT to all agreements, conditions, easements and restrictions of
record and to the provisions, easements, covenants and restrictions as contained in the
Declaration of Condominium, Code of Regulations and Declaration Plans.

BEING the same premises which Richard L. Prove and Margie A. Prove, his wife, by
their deed, dated July 28, 1989 and recorded August 1, 1989 in the office of the Recorder of
Dceds in and for Cumberland County, Pennsylvania in Deed Book B34, Page 1136, grantcd and
conveyed unto Anthony Dolimpio, single man, said grantor hercin.
TOGETHER with all and singular the buildings and improvements, ways, waters, water-
courses, rights liberties, privileges, hereditaur-ents and appurtenances and whatsoever thereunto
belonging or in anywise appertauiing, and the reversions and remainders, rents, issues, and profits
thereof; and all the estate, right, title, interest, property, claim and demand whatsoever of the said
party of the first part, in law, equity or otherwise, howsoever, in and to the same and every part
thereof.
TO HAVE AND TO HOLD the said hereditaments and premises hereby granted or
mentioned, and intended so to be, with the appurtenances, unto the said party of the second part, his
heirs and assigns, to and for the only proper use and behoof of the said party of the second part, his
heirs and assigns forever.
The said party of the first part, for his heirs and assigns, do by these presents covenant, grant
and agree to and with the said party of the second part, his heirs and assigns that tbey the said party
of the first part, his and assigns, all and singular the hereditaments and premises herein above
described and granted or mentioned, and intended so to be, with thc appurtenances, unto the said
party of the second part, his heirs and assigns against them the said party of the first part and his
heirs and assigns, and against all and every other person or persons whomsoever lawfully claiming,
or to claim the same or any part thereof sha11 and will specially warrant and forever defend.
